Who is authorized to sign for the release of my health information?

The following individuals are authorized to sign for release of your health information:

  • You, the patient
  • Your health care directive designee or health care power of attorney, if applicable (health care directive document must be provided)
  • Parent, if the patient is younger than age 18
  • Legal guardian (proof of guardianship document must be provided)

How much does it cost to obtain a copy of my health information?

  • Patients are not charged for records released directly to themselves
  • There is no charge for releasing copies of medical directly to other healthcare providers, clinics, or hospitals.
  • Copies of medical records for parties other than patients and healthcare providers (including law firms, insurance companies, and other business requestors) will be charged processing fees, per page and/or CD fees, sales tax if in MN, and shipping and handling fees based on the Department of Labor’s annually adjusted postage fee.

When will I receive a copy of the medical records?

Requests are processed as soon as possible, are prioritized based on need and date requested, and completed within 30 days from date request was received.
