WHAT IS THE CMS GUIDE PROGRAM?
The CMS GUIDE Program (Guiding an Improved Dementia Experience) is a national initiative from the Centers for Medicare & Medicaid Services. GUIDE offers comprehensive, evidence-based support for individuals with dementia and their caregivers.
HOW MCN IS PARTICIPATING
Minneapolis Clinic of Neurology is proud to partner with Craniometrix for our new GUIDE Program.
This program offers benefits such as:
Care Coordination
A care navigator helps keep your care connected
Respite Coordination
If eligible, caregivers can access respite care at no additional cost
CheckIns & Ongoing Support
Regularly scheduled phone call check-ins from care navigators
24/7 Support & Advice
Access to a 24/7 support line for urgent concerns and guidance anytime
Caregiver Education & Resources
Access to a 24/7 support line for urgent concerns and guidance anytime
ELIGIBILITY REQUIREMENTS
To qualify for CMS GUIDE:
• Enrolled in Original Medicare (Parts A and B); not Medicare Advantage or PACE
• Have a confirmed dementia diagnosis
(e.g., Alzheimer’s, vascular dementia, Lewy body dementia, frontotemporal dementia)
• Not currently in hospice care or reside full-time in a nursing home
