To schedule or change an appointment, call your preferred clinic at the number below.
Booking Your Appointment:
- New Patients: New patients or if you have not been assigned a specific staff member, please call the office you prefer. Describe your concerns so we can allocate adequate time with the appropriate specialist.
- Existing Patients: If you are currently seeing one of our specialists, please contact that specific office directly to schedule or change your appointment.
- Request Appointment: To request an appointment, please fill out the following form.
Preparing for Your Visit:
- Arrival Time: New patients should arrive 10-15 minutes early to complete registration, which includes demographic and insurance information collection. Even if pre-registered by phone, please arrive early to sign forms and complete medical history.
- Follow-Up Visits: Arrive a few minutes early to verify and update any information and make co-payments if required.
- Cancellations: Please inform us 24 hours in advance if you need to cancel your appointment. This allows us to offer the slot to other patients. A fee of $50 may be charged for missed appointments without 24-hour notice.
Your Medical Care:
- Promptness: Arriving on time for your appointment minimizes wait times and helps us provide better service to all patients.
- Insurance and Payments: Always bring your insurance card and a photo ID. Co-payments are collected at each visit as required by your insurance. A $20 service charge will be applied for unpaid co-payments to cover billing costs.
- Feedback: Your comments and suggestions are vital. For any concerns, please speak to a staff member or call our Administrative Offices at (763) 302-4100.

- Q: Do I need a referral to see a neurologist?
- A: Yes, a referral is necessary to ensure appropriate specialist care and timing. Note that a referral is different from an insurance authorization.
- Q: What information is needed for my appointment?
- A: Please provide a referral letter, diagnostic test results, and your medical history including current medications at least a week in advance if possible.
- Q: How can I prepare for my appointment?
- A: Write down your questions, bring all medications, take notes during the visit, and consider bringing a friend or family member for support.
Medication Policy:
- For refills, contact your pharmacy well before your medication runs out. Your pharmacist may need to consult with your neurologist, especially for controlled substances.
Hospital Inpatient Care:
- Discuss with your physician the most suitable hospital for emergencies or scheduled care. We provide inpatient neurology care at major and community hospitals in Minnesota and western Wisconsin. View our affiliated hospitals.
Emergencies:
- Life-threatening: Call 911 immediately, then contact your neurologist.
- Urgent care during business hours: Call your neurologist directly.
