Jacob “Jake” Forsythe is a board-certified Adult-Gerontology Acute Care Nurse Practitioner with clinical interests in migraines, headaches, cervical dystonia, and stroke. Before becoming a nurse practitioner, Jake worked in several neurology settings, including intensive care, which helped build his long-standing interest in neurological care.
Jake joined the Minneapolis Clinic of Neurology because he wanted the opportunity to expand his experience in neurology and care for patients from a broader provider perspective. He believes in taking a holistic, individualized approach to care, recognizing that no single treatment plan is right for every patient. Listening to patients and involving them in decisions about their care are important parts of his practice.
One experience that reinforced Jake’s proactive approach involved a patient being evaluated for arm pain. After ordering additional imaging and recommending further evaluation when unexpected findings appeared, the patient was ultimately diagnosed with multiple sclerosis and was able to begin appropriate treatment.
Outside of work, Jake enjoys biking, hiking, canoeing and kayaking, traveling, reading, cooking, and watching movies and television. His approach to personal health centers on moderation, staying active in ways that are enjoyable and sustainable, and using meditation and breathing techniques to help manage stress.
